To set up your profile on the La Vie website, set up your email account, and give you access to the Blue Book room scheduler, please fill out the form on this page. To fill out the form in its entirety, please gather the following information ahead of time:
- A brief bio, maximum 1,000 words
- Your license or pre-license status: Licensed, Associate MFT, MFT Trainee, LPC Trainee or Associate, or Psych Associate – IMPORTANT: Your profile description must be approved by your supervisor. Please obtain his or her approval before filling out this form.
- California License or Associate Number, if applicable
- A recent digital full-color picture (at least 250 pixels by 250 pixel or larger)
- Your phone extension at La Vie
- Areas of Clinical Interest
- Modality (couples, individuals, etc.)
- Hourly fee
- Whether you will take insurance or not
- Whether you will offer a sliding scale or flexible fee
- Whether you plan to practice in Pasadena, or via Telehealth, or both.
